One lost money before anyone realized what was happening. The other caught it in time. Neither of them saw it coming, and both of them had something in common: their exposure lived inside a form on their website that anyone on the internet could reach.</p><p>The attack pattern is straightforward. Automated bots flood a web intake form, submitting fraudulent applications with convincing falsified documentation. Fake leases, fake IDs, fake paycheck stubs. The submissions look real enough that payments go out before the fraud is identified. The median time from a software vulnerability being disclosed to the first working exploit has fallen from 771 days in 2018 to effectively zero in 2025, with the majority of exploited vulnerabilities now weaponized before they are even publicly disclosed. A 2024 study documented that AI-assisted exploit generation costs $8.80 per working exploit. That number has only dropped since. Not because they ignored risk, but because they misunderstood what matters first. &#8220;It&#8217;s not about if a cyber incident will happen,&#8221; he said. &#8220;It&#8217;s when. And what you do in the hours after matters more than most people realize.&#8221;</p><p>The simulation exposed a truth that rarely survives into marketing decks. Cyber incidents do not arrive with clean edges. Loss does not announce itself politely. Cyber insurance works, but it moves at insurance speed. For many small and mid-sized businesses, that delay is where the real damage lives. Rent does not wait. Employees do not wait. Vendors do not wait. </p><p>&#8220;When you fall down,&#8221; Wolf said, &#8220;how do you get back on your feet financially while everything else is still being figured out?&#8221;</p><p>That question hung over the rest of the discussion. In the first hours of an incident, theoretical coverage matters far less than liquidity. Payroll still runs. Forensics and legal counsel do not wait for reimbursement cycles. When cash tightens, even well-prepared businesses start making decisions they regret. The simulation made clear how quickly financial pressure, not technical failure, becomes the dominant risk.</p><p>One real-world example drove the point home. A construction firm with solid security controls paid what looked like a routine invoice. Weeks later, they learned it was fraudulent. The cyber insurance policy excluded wire transfer fraud. The bank could not recover the funds. The loss sat unresolved until outside capital stabilized the business. Nothing about the attack was dramatic. The financial impact nearly was.</p><p>Runyan was direct about the insurance side of that equation. &#8220;Not all cyber insurance is created equal,&#8221; he said. </p><p>Policies differ in what they cover, how they pay, and when they respond. Some reimburse after the fact. Others pay on behalf. Some exclude losses businesses assume are included. Those differences live in policy language that rarely gets attention until it becomes decisive.</p><p>The conversation kept circling back to one control that cuts through nearly every scenario: multi-factor authentication.</p><p>MFA is no longer a differentiator. It is the floor. Insurers now enforce it with little patience. If a business attests that MFA is enabled and forensic evidence later shows it was not, claims can be denied outright. That is no longer theoretical. </p><p>&#8220;That&#8217;s not a mistake,&#8221; Runyan said. &#8220;That&#8217;s a misrepresentation.&#8221;</p><p>Yet even strong controls no longer buy certainty. The baseline has moved. As underwriting tightens, insurers are paying closer attention to how quickly an organization can detect and contain an incident once it begins.
